My Health For Life Program
Report
My health for life program is a fabulous opportunity if you are want to live well and stay healthy. My health for life is paid for by the QLD Government and is FREE for eligible participants residing in Queensland. There is no means test for this program. To know if you qualify, all you need to do is complete a health check online that only takes a few minutes.
Diabetes Programs

We know how overwhelming it is to receive a diabetes diagnosis, and it can take time to work out what to do. Evidence shows that the sooner you can make the correct dietary and lifestyle changes, the more likely you are to control your diabetes without needing medications, or at least minimal medications.
8 Week Diabetes Group Challenge
Report
Initial individual consultation to collect baseline data and measurements and get to know you. Initial Assessment includes your current concerns, relevant medical history; height and weight and waist measurements; current fitness level-this must be completed before group session start. We explore the causes/drivers of diabetes concerning lifestyle factors and diet, including food choices, physical activity, stress, sleep, and genetics.
Diabetes Essentials Program

Nutrition Assessment includes identifying your current concerns; diet history, including food preferences; relevant medical history; height and weight and waist measurements; lifestyle balance; and digestive function.

A nutrition plan includes your estimated requirements for nutrients (energy, carbohydrates, protein, fats, fibre and fluids), to meet your nutritional needs and goals; and recommendations for any lifestyle changes that will improve diabetes.
