Prior to your initial consultation you will be asked to complete a new patient information and medical history questionnaire. Following discussion and review of your information, we will undertake a comprehensive examination. This includes (but is not limited to) a soft tissue assessment, an examination of your jaw and a complete tooth charting and record.
