Please arrive 10 minutes earlier so that you have enough time to complete a comprehensive medical and dental questionnaire. We are Bupa and HCF preferred provider. Mostly private health insurance health funds rebates are different and may require out of pocket or gap payments. Chapel Gate Dental supports 'HICAPS' so that you can settle 'gap' payments on site.
